The Department of Justice Gambling Control Division — which licenses the video gambling machines, live card games and sports pools in Montana's bars and casinos — publishes a Problem Gambling page that offers only helplines ('Help is available in many Montana communities. Covers online betting
Not confirmed. Montana law requires the State Lottery and Sports Wagering Commission's rules to cover 'player exclusion requirements' for sports wagering (23-7-202(1)(g), MCA), and Sports Bet Montana is the only legal sportsbook in the state. But neither the Lottery nor the Gambling Control Division publishes a self-exclusion form, terms or any description of how that exclusion works, so we could not confirm what it covers or how to use it. Ask the Montana Lottery directly before relying on it. No state list covers them. No. The Montana Lottery's own Play Responsibly page describes no self-exclusion or voluntary exclusion option at all. Worth knowing before you start

Montana is a state where the thing people assume exists does not. There is no list to join, so the levers that work here are the ones you control: ask each venue you use to bar you (the casinos and tribal casinos keep their own exclusion lists and handle these requests at the door or the responsible-gaming desk — ask that casino, because the state keeps no register of it and the Division says it 'does not regulate enforcement decisions' of that kind), set an exclusion inside your Sports Bet Montana account, close betting accounts one by one, and put a gambling block on your card or bank account. The Division enforces state-tribal gaming compacts but licenses no tribal casino player exclusion list we could find, so never assume an exclusion at one casino reaches another.
