Covers online betting
Yes — casino-linked online gambling is included. The form states the exclusion 'is valid for ALL West Virginia Casinos, including the Historic Greenbrier Casino, and all mobile applications and gaming associated with West Virginia Lottery', and you agree not to 'access any online gaming applications from which I have requested exclusion'. The FAQ goes further than most states: 'The exclusion will apply across all domains and brands conducted by the same operator. For example; self-excluding from Hollywood Casino in Charles Town, W. Covers the lottery
No, and the form says so in terms: 'I understand that Self-Exclusion from Casinos, iGaming, and Sports betting does not exclude the player from iLottery (iPlay). That must be done via the Self-Exclusion program on the mobile app or website.' How long you can choose
There is no choice of term — it is for life, with the chance to ask out after a year. The FAQ asks 'How long is a patron self-excluded from casinos and mobile gaming applications?' and answers: 'A patron is excluded from casinos and mobile gaming applications for life. Can it be undone
Only by applying after a year, and only if the Commission agrees. The form states: 'The West Virginia Lottery Commission will consider removal from the statewide Self-Exclusion List after one year from the date of the exclusion', that you must 'submit a written request to the West Virginia Lottery using its Self-Exclusion Removal Form', and that 'I will be on the list permanently unless a determination is made by the West Virginia Lottery Commission, or a court of law, to the contrary.' What happens when it ends
It never expires. There is no term to run out: the listing is permanent — 'I will be on the list permanently unless a determination is made by the West Virginia Lottery Commission, or a court of law, to the contrary' — and the Commission will only consider removal once a year has passed since the exclusion, on a written application using its Self-Exclusion Removal Form.Nothing to diarise and no renewal to miss, which is exactly what most people in this position want. The separate iLottery (iPlay) self-exclusion is a different matter: the Lottery describes that as something you may set 'temporarily or permanently' in the app, so if you use the temporary option there, that one can lapse. How you sign up
On paper, by post or in person — there is no online enrolment. 'Must be notarized if this form was not completed in the presence of West Virginia Lottery Personnel', and you must 'copy and attach photo identification used to verify patron's identity'. The FAQ lists what the form asks for: the last 4 digits of your Social Security number, date of birth, front and back of your driver's licence, current address, phone number and a brief description of the patron requesting. Worth knowing before you start

West Virginia is one of the strongest state schemes in the country for someone who wants to stop for good: the default is life, there is no term to pick and nothing lapses on an anniversary, the ban reaches casinos and the operators' online and mobile products together, and it follows a brand across state lines — self-excluding from Hollywood Casino in Charles Town also excludes you from Hollywood Casino in Columbus, Ohio. Three things to know before you post the form. First, it is genuinely hard to undo, which is the point, but it means a year of waiting and then a Commission decision at a monthly meeting, with no guarantee. Second, there is a real privacy cost that catches people out: 'I understand that the West Virginia Lottery Exclusion List is public record, and therefore my inclusion on this list is public record.' If being named on a public list is a problem for you, weigh that before enrolling — and know that it is the trade for a ban casinos actively enforce, since 'casino surveillance operations and the West Virginia Lottery take reasonable measures to identify self-excluded persons who attempt to enter the Casinos'. Third, iLottery is not included. Excluding yourself from casinos, iGaming and sports betting leaves iPlay open, and you have to go into the iPlay app or website and use its own self-exclusion under Limits in My Account. The Lottery also warns you remain 'ultimately held responsible' for limiting your own access, and that you cannot hold it or the casinos liable if you do get in.
